statement for the protection of all retained trees, shrubs and
watercourses on, or adjacent to, the site from any physical or
chemical disturbance or pollution during all site operations shall
be submitted to and agreed in writing with FRM. The method
statement shall include the use of protective fencing as per
BS:5837 2012 and shall thereafter be fully implemented.
f) The works shall be commenced within 3 years of the date of the
approval. Should the works not be commenced within this time frame,
the current approval will be deemed to have lapsed.
g) The applicant shall provide 7 days notice to FRM of their
intention to commence the works; and
h) As-built, drawings shall be submitted to FRM on completion of
the works.

It is recommended that the Director of Public
Health -
(i) Waives Contracts Procedure Rules 8.1 and 8.2 to extend the
contract with Asda supermarket for a period of 12 months to pilot
an additional model of delivery for the NHS Health Checks Programme
in Leeds incurring a maximum contract value of £90,000.
(ii) Vary the GP Healthcheck contract for one year (with provision
to extend for a further year) to adjust the current patient
invitation payment to GP’s to compensate them for the
additional work involved in inviting patients to use the Asda
service and in updating their patient records afterwards incurring
a maximum additional cost of £41,400.
Leeds was approached by Public Health England and given the
opportunity to work in partnership with Asda supermarket to pilot
for 6 months a model whereby the NHS Health Check would be
delivered through Asda pharmacies within 4 Asda supermarkets across
Leeds which are Morley, Seacroft, Pudsey and Holt Park. This would
be in addition to the current GP providers. These sites were chosen
as they are the only Asda supermarkets with a pharmacy in store and
this is a requirement to deliver the NHS Health Check.
This additional method of delivery of NHS Health Checks will give
people more choice and flexibility to access an NHS Health Check
which in the longer term will enable people to adopt healthy
lifestyles and therefore reduce their risk of developing
Cardiovascular Disease and dying prematurely.
Due to delays implementing the initial 6 month pilot, patients were
not able to benefit from the alternative provider of Health Checks
until mid-November. The number of patients accessing the new
service has increased since the pilot has been operating, which
shows that there is public interest for an alternative provider. It
is now proposed for the pilot to be extended for a further period
of 12 months to allow more people to access the Health Check
service through Asda pharmacies. This extended pilot period will
enhance the evaluation of the pilot, and will help inform and
strengthen future commissioning intentions.
This contract extension and variation will be funded by the
established budget for the delivery of the NHS Health Check
Programme, which is part of the ring fenced public health grant
which transferred to LCC Public Health from the NHS in April
2013.

service provision and the decision to undertake a procurement
exercise. It then outlines the procurement process and the outcome
of the evaluations.
The open procurement process was followed which commenced in
December 2014 when the documents were published through
YORtender,
The Pre-Qualification Questionnaire (PQQ) and Tender Documents
Parts 1 to 4 were issued at the same time and bidders were given
the deadline of 28th January 2015 to submit PQQ’s and tender
submissions.
The quality criterion in the invitation to tender was set at 100%
quality. There were qualitative areas that tenderers had to respond
to, which focused on:
• The design, development and implementation of the
programme
• Involvement of people in the delivery and evaluation of the
programme
• Prioritisation of newly diagnosed patients onto the
programme
• Communications Strategy and promotional strategy
• Competence of the educators
• Accessing and supporting potentially vulnerable groups
• Understanding of community development principles
• Support mechanisms to ensure attendance on the
programme
An evaluation panel, consisting of commissioning officers with
experience and knowledge relating to Long term conditions and self
management evaluated the four tender submissions in line with the
evaluation model set out in the tender instructions.
Due diligence checks of the submissions for this contract have been
undertaken by the Council’s PPP&PU. These included the
uptake of references from bidders, ensuring that the appropriate
levels of insurance were in place, an inspection of all audited
accounts as well as health and safety policies, information
governance and safeguarding.
The new contract is to commence from 1st April 2015 until 30th
November 2015, with no option to extend.
Consultation and Engagement
Significant consultation has been undertaken on the service
specification. The tender documentation was subsequently developed
to ensure that the key issues were reflected.
A Consultant in Public Health, A Health Improvement Principal, and
two GP’s were on the evaluation panel, all of whom have a
vast wealth of knowledge and experience in this field.
Equality and Diversity / Cohesion and Integration
An Equality Impact Assessment screening tool has been undertaken
and is attached at Appendix 2.
Appropriate policies and procedures are in place by the recommended
provider and were reviewed as part of the PQQ screening
process.
Resources and value for money
The value for the proposed contract award is £185,000 which
is a fixed budget for the provision of this service. This is
reflective of current service funding and does not introduce any
additional funding pressures.
The contract is being awarded in the sum of £185,000, which
is the fixed budget for the delivery of this service.
The Health Improvement Principal will support implementation of the
contract award to ensure the new service is in place and
operational by 1st April 2015.
