Consultation process

The local strategy for timely transfers of care has been developed via Leeds Dementia Partnership, including a multi-agency workshop event, a Steering Group meeting fortnightly at St James Hospital, and meetings with care home providers and Leeds Care Association.  The proposed decision is to enable joint investment to implement the strategy.
