Brief Description

Community services commissioned or provided by adult social care play an important role in supporting system flow. Adult Social Care is also continuing to innovate and transform in order to provide better quality services, improve staff working conditions and improve efficiency in community-based services.

There are a number of change programmes:

·  Establishment of a Transfer of Care hub

·  Developing an integrated offer between SkILs reablement and Leeds Community Healthcare Trust Neighbourhood Teams

·  An Intermediate Care services strategic review

·  Creation of a Community Health and Well-being Service

Leeds NHS organisations have made available £4m non-recurrent funding to:

1.  Support the introduction of a minimum wage of £10.50 per hour for commissioned home care services for 2022-23.

2.  Support the creation of a Community Health and Well-being Service.

The Director of Adults and Health will be asked:-

To note that all further decisions taken that these funds are used for will be direct consequences of this key decision and will be either Significant Operational Decisions or Admin Decisions, as per the nature of each decision.

Financial and procurement implications

Other relating work to continue to innovate and transform adult social care will be from the £2m resource given to the Council by Leeds NHS organisations to support the development of integrated services.

Partnership working

Leeds City Council will take the lead on deciding how the £4m resource will be utilised but this is in discussion with Leeds NHS partners around specific issues such as (a) support to the home care sector and (b) development of the Community Health & Well-being Service.
