To fund through government grant, the development and roll
out of a 'Person Held Record' (PHR) platform initially for the
peopleof Leeds. This programme will provide citizens with
secure and verifíed access to a more holistic view of their
health, care and wellbeing information. ln addition, it will also
providethe capability for citizens to contribute information to
their own record.
