Local Care Partnerships are being developed across Leeds to deliver health and care services in a way that works for local people. All Local Care Partnerships share the same key feature –a range of people working together, regardless of the employing organisation, to deliver joined up care that meets the needs of each local population. Local leaders of Health and Care services meet regularly with other partners including housing and leisure to develop ways of working that support local people to live healthy, active lives and to address wider determinants of ill health. Working in and with local communities, Local Care Partnerships have an ambition to improve health and well-being for all members of the local community. The Armley Care Partnership covers the areas of Armley, Bramley, Wortley and parts of Middleton.
