Purpose:

The key decision is for new contracts arrangements for the family healthy living services to be put in place from 1st April 2019.  The contracts are with 3 local providers (Leeds Community Healthcare NHS Trust, Health for All and DAZL and deliver a range of children and family weight management, physical activity and healthy eating services.  The annual value of these services is £274,060.

 

Decision:

The Director of Public Health approves the 12 month extensions from 1st April 2019 of contracts held with existing third sector providers (Health for All (DN299684) and DAZL (DN230289)) for the delivery of family and children’s healthy living services as defined in Appendix 1 under contract procedure rules (CPR) 21.1.

 

The Director of Public Health approves a variation to 0-19 Public Health Integrated Nursing Service contract (DN336699) with Leeds Community Healthcare NHS Trust to include the child healthy weight management service as defined in Appendix 1 in accordance with contract procedure rules (CPR) 21.7.
